Forms

The following forms are required for participation by all Atomic Juniors and Masters rowers. Please read, complete, and return these forms to our Athletics Director as soon as possible. The forms are completed once for your time with Atomic Rowing, unless noted otherwise.

*To be eligible for Regional and National competition, athletes are required to join USRowing as a Championship Member. During this process, you are required to sign a USRowing waiver, and affiliate with Atomic Rowing, Club Code: NNWKN
